Welcome to Five Starr Pilates & Fitness!
The success of any Pilates program depends upon continuity, presence and accountability. The Five Starr Pilates & Fitness Client Intake package ensures a strong foundation enabling you to reach your goals and perfect your technique. The intake forms present the guidelines for fees, scheduling, cancellation, punctuality and safety. All of these elements are important issues to anyone receiving professional services at Five Starr Pilates studio. Most people can find excuses to avoid exercise. By upholding the foundation of this contract, you agree to honor your own progress. These policies are strictly enforced. To help our studio run efficiently, please note the following policies.
Payment Policy
The studio accepts cash, checks and Visa or MasterCard. There is a $35 fee for returned checks. No refunds.
Safety
If you have a change of physical condition or new medication, it is important to let the instructor know ahead of time so that appropriate precautions can be taken. Likewise, if safety hazards present themselves during a session/class, your session/class may be halted until the problem is resolved. You are obligated to inform the instructor of any pain, discomfort, fatigue or any other symptoms that you may suffer during and immediately after participation. You may stop or delay participation in any activity if you so desire, and you may also be requested to stop and rest by the instructor who observes any symptoms of distress or abnormal response. The session/class is forfeited without refund if you are under the influence of drugs or alcohol during a session/class.
Packages Purchased
All sessions are booked in advance. Payment is expected in full before or on the date of the first session. Payments are non-refundable and expire within 3 months of initial purchase. To extend the expiration date of a package due to expire, we will extend the date if a package of the same size or larger is purchased. No refunds.
Memberships
All memberships carry a 4-month minimum. This means the membership cannot be canceled until 4 months have been fulfilled. There is no cooling-off period. Memberships may be canceled with a 30-day written notice. The 30-day period must be 30 days from your next scheduled billing date. So if you are due to bill on the 7th of each month, then you must give your written notice 30 days before the 7th. You may cancel your membership by emailing the 30-day notice to manager@fivestarrpilatesandfitness.com. All final amounts due will be collected when the 30-day notice is received and processed by our management. No refunds. Session Cancellations
Sessions cancelled without 12-hour notice are forfeited. Sessions cancelled with 12-hour notice may be rescheduled. Cancellations may be done by telephone or using our online booking system. Memberships will be charged a $10.00 Late Cancel or “No Show” fee for late cancellations or “No Shows”. Clients using packages who cancel without a 12-hour notice forfeit the class or session.
Punctuality
More than 5 minutes tardiness constitutes a late cancellation in a Pilates class and 5 minutes in a cycling class as well. For private training, the instructor is not obligated to conduct the session if you are more than 5 minutes late. In some cases, the instructor will wait and work with you for the remaining time. However, this is solely up to his/her discretion. In either case, you will be charged fully for the session. The instructor is not obligated to make up time due to lateness.
